Travel Clinic

If you require any vaccinations relating to foreign travel you need to:-

  • Complete the Travel Questionnaire which is downloadable from the website or you can collect a form from the surgery.
  • Hand the completed form to the receptionist who will make an appointment with either Nurse Norma or Nurse Bola to discuss your travel arrangements.  This will include which areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible  –  at least 3 weeks before you travel.

Some travel vaccines/medications are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because travel vaccination is not included in the services provided by the NHS and therefore is a chargeable service.

To help us offer the appropriate advice, please fill out the form below and either bring it into the surgery or email it to lamccg.odhc@nhs.net.  The receptionist will then book you an appointment, allowing time for the Nurse to look at your form before you come to see her.  She may telephone you for more information prior to the appointment date.
